Access to Counseling Records
Clients have the right to request access to their counseling records in accordance with Texas law. To request copies of your records, please submit a request through the contact form available at: www.suzettelamb.com
Requests must include sufficient information to verify identity. Record requests will be processed in accordance with applicable state and federal laws. Reasonable fees may apply as permitted by law.
